We present a case of variation of origin and tendon disappearance in a 58-year-old Korean male cadaveric specimen in our routine dissections. First, the origin of the plantaris muscle is slightly medial above the lateral articular eminence of the lateral head of the gastrocnemius muscle. That is, compared to the normal origin, it started from the infero-medially side. Second, we observed a variation in which the tendon thins and disappears without being fused to the gastrocnemius or soleus muscle at the proximal 1/3 of the calf during the normal course. The disappearance of the plantaris tendon during the course is extremely rare. It would be an anatomical basic data that could be applied during tendon transplantation clinically or that can find the developmental sequence of plantaris muscle.
